JAMA Surg 2025; 160: 783–793.
This multicenter study of 173 patients with rectal cancer <10 cm from the anal verge compared total mesorectal excision with chemoradiotherapy followed by local excision (TME group) against chemoradiotherapy and transanal endoscopic microsurgery (TEM group). At 5-year follow-up, local recurrence (primary outcome: 7.4% TEM vs 6.2% TME), as well as distant recurrence, overall and disease-free survival were similar between the groups.
Comment: This study showed equivalence of these procedures, but the study was relatively small and did not include quality of life data.
