Long-term outcomes of early surgery vs endoscopy first in chronic pancreatitis. Follow-up analysis of the ESCAPE randomized clinical trial.

This study examined the outcome of this RCT after eight years; 86 patients were available for analysis. The early surgery cohort had lower Izbicki pain scores (33 versus 51) and were more likely to have complete pain relief (45 versus 20 per cent). Around half the early endoscopy group eventually had surgery, and they had the worst outcomes.
Comment: The most interesting finding was that a strategy of trying endoscopy and then proceeding to surgery if that didn’t work would seem to be unwise.
