Of 203 patients, those in synthetic mesh group had lower midline hernia recurrence (hazard ratio 0.46, 95 per cent confidence interval 0.25 to 0.86, P=0.015). At median follow-up of 5.4 years, the absolute risk reduction for midline hernia recurrence was 11.8 per cent (2.1 to 21.4) with synthetic mesh, with no significant difference in recurrence rates or mesh-related complications beyond 2 years.
Comment: Synthetic mesh reduces midline hernia recurrence without increasing mesh-related complications in clean-contaminated and contaminated repairs.
