Balanced crystalloid solution versus saline in deceased donor kidney transplantation (BEST Fluids): a pragmatic, double-blind, randomised controlled trial.

Collins MG, Fahim MA, Pascoe EM, Hawley CM, Johnson DW, Varghese J et al, on behalf of the BEST-Fluids Investigators and the Australasian Kidney Trials Network.

Lancet 2023; 402: 105-117.nced

Some 808 patients having kidney transplantation from deceased donors were included. Delayed grafted kidney function was reduced by the use of balanced crystalloid fluids for 48h after transplantation: 30 per cent, compared to 40 per cent with saline.

Comment: The authors state this should be the standard of care. Hard to disagree.
